to Dr. Wiedershine to request In-Take and Consultation forms.
2. Check your Email
You will receive a two-page Intake Form and a one page Consultation
Agreement Form. Directions will be given for you to return the forms
by fax or by mail.
3. Set a Convenient Time
Dr. Wiedershine will contact you by email or phone to arrange for a
consultation at your earliest convenience. Once the time is set, you
will call Dr. Wiedershine at a number the office will provide.
NOTE: While the preferred appointment method is by email, those without internet access may set up an appointment by calling 845-679-2407. Dr. Wiedershine's consultation fee is $200 per hour and can be paid by credit card at the time of your appointment.
